Shift handovers in powder plants follow a predictable script:
Outgoing operator: "Everything's fine. Had a small alarm on the sifter around 2 AM, but I reset it."
Incoming operator: "Cool. See you tomorrow."
Plant Manager: "Any issues overnight?"
Both: "No, all normal."
Here's what actually happened between 2:00 and 2:15 AM:
None of this made it into the log. Because the operator didn't see it. And even if he did, he wouldn't know how to describe it.
It's not laziness. It's a system problem:
Most powder equipment doesn't give the operator any feedback. No temperature gauge on the bearing. No vibration indicator on the sifter. No visual flow indicator on the cone mill. If the machine is running and the alarm light is off, it must be "normal."
If nobody has ever shown the operator what a healthy bearing temperature feels like, how would he know when it's wrong? If he's never seen a properly tensioned screen, how would he know when it's loose? "Normal" becomes whatever the machine did yesterday.
Operators know that writing "vibration on sifter" might trigger a work order, which might make their shift look bad. So they write "all normal" and hope the problem goes away. The logbook becomes a CYA (Cover Your Ass) document, not an operational record.
Every operator has his own mental checklist—if he has one at all. One guy checks the bolts. Another guy checks the pressure gauge. A third guy checks nothing. The handover is whatever each person decides to share.
You can't train your way out of this problem. You can't write a better SOP. You can't threaten operators into filling out logbooks more carefully.
